Sign Waiver
Every guest must sign this liability waiver before participating in a 4 Paws & Peaks experience. Please read the agreement carefully, then complete and submit the form below. You'll receive an on-screen confirmation when your waiver is recorded.
Liability Waiver, Release of Claims & Assumption of Risk
By signing below, I ("Guest") acknowledge and agree to the following in connection with any guided hiking, trail, or outdoor experience offered by 4 Paws & Peaks ("the Company"), its owners, guides, employees, contractors, agents, and affiliates (collectively, "Released Parties").
1. Assumption of Risk. I understand that outdoor activities in Utah involve inherent and significant risks — including uneven terrain, falls, slips, rockfall, exposure to weather, altitude, wildlife, interactions with dogs, illness, serious injury, and death. I voluntarily accept and assume all such risks, whether known or unknown, foreseen or unforeseen, for myself, anyone in my party (including minors), and any dog under my care.
2. Release and Waiver of Liability. To the fullest extent permitted by law, I hereby release, waive, discharge, and covenant not to sue the Released Parties from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, injury, illness, emotional distress, property damage, or death sustained by me, anyone in my party, or any dog while participating in any Company experience — including, without limitation, claims alleging negligence by the Released Parties.
3. Indemnification. I agree to indemnify, defend, and hold harmless the Released Parties from any loss, liability, damage, or cost (including reasonable attorneys' fees) they may incur arising out of my participation, my conduct, or the actions of any dog under my care.
4. Dog Owner Responsibility. I represent that any dog I bring is healthy, current on vaccinations (including rabies), free of communicable illness, reasonably socialized, and under my control at all times. I am solely responsible for any injury, damage, or harm caused by my dog to any person, animal, or property.
5. Medical Authorization. I authorize the Company to seek and obtain emergency medical or veterinary care on my behalf if needed, at my sole expense.
6. Photography & Media. I grant the Company permission to use photographs or video taken during the experience for marketing, unless I have opted out in writing prior to the experience.
7. Governing Law. This agreement is governed by the laws of the State of Utah. If any portion is held unenforceable, the remainder shall continue in full force and effect. Any dispute shall be resolved in the state or federal courts located in Utah.
8. Electronic Signature. I understand that typing my full legal name below and submitting this form constitutes my electronic signature and has the same legal effect as a handwritten signature under the Uniform Electronic Transactions Act. My signature, IP address, browser information, and the date and time of signing will be recorded for our records.
